Dark circles or bags under the eyes?
Both make you look tired, but the treatments are fundamentally different. Dark circles are a depression or discoloration under the eye: The tear trough deepens over the years, and the thin skin allows blood vessels and muscle to show through, creating a shadow when light hits it. Eye bags are the opposite, a protrusion of fatty tissue that persists even after a restful vacation. We fill in dark circles, while eye bags require surgery; see lower blepharoplasty. Often, both conditions are present at the same time; in such cases, we plan treatment in two stages or as a combined procedure. Lightening Dark Circles Under the Eyes: First the Cause, Then the Method
Dark circles under the eyes can have three possible causes, and each requires a different solution. If the dark circles are caused by a loss of volume (the groove between the lower eyelid and the cheek), filling the area with hyaluronic acid or autologous fat can help. If it’s a bluish discoloration caused by very thin skin that allows blood vessels to show through, dermal fillers are also effective, supplemented by procedures that firm the skin, such as microneedling. If it’s a brownish pigmentation, fillers won’t help; in this case, we use skin-lightening treatments and consistent sun protection. During the consultation, we determine the specific type by stretching the skin and using varying light conditions.

For the first step, we usually recommend hyaluronic acid: it allows you to see, without any risk, how much volume the eye area can handle. If you like the result and want it to be permanent, you can switch to autologous fat.
Dark Circles Under the Eyes in Men
Men often have dark circles under their eyes, but rarely talk about them. The treatment is the same, but the dosage is more conservative so that the eye area doesn’t look soft or “artificial.” A deep shadow caused by a pronounced tear trough is typical in athletic, slender men; in these cases, just a few tenths of a milliliter of hyaluronic acid per side is often sufficient. There is no downtime; you’ll be back at the office the very next day.
